There is grain whether the adpater is on or off.

First of all, make sure you are setting your focus on the ground glass with your camera zoomed ALL the way in and no 35mm lens attached. Point the adpater at a bright object and focus until the ground glass looks like rough sand paper. When this is achieved. Leave the focus alone and use gaffers tape to lock your camera's focus ring in place. Zoom back out to fill the frame optimizing as much or as little of the projected image from the 35mm lens as you wish.
Other things to check...
Auto focus on the camera is turned OFF.
Gain setting is set to ZERO or lower.
Camera aperture is wide open or near to it.
Aperture on the 35mm lens set to f5.6 or wider.
Shutter speed set under 1/120
